The SteadyGuide Plan provides steady, professional support over one year for individuals and families navigating changes in health, aging, or roles. It helps you notice meaningful shifts early, keep everyone aligned, and avoid starting over every time circumstances change.

This plan is designed to offer consistent, calm guidance — not crisis response — and it focuses on shared understanding, not clinical intervention.

Faoundations Package

Foundational Family Snapshot (once)

A structured family conversation that documents:

  • who is involved

  • roles and responsibilities

  • current understanding and areas of alignment or uncertainty

This creates a shared baseline early in the relationship.

SteadyGuide Touch-Bases (12 per year)

Monthly 45-minute check-ins focused on:

  • monitoring meaningful changes across the 4Ms (What Matters, Medications, Mentation, Mobility)

  • capturing family-reported signals

  • maintaining continuity and shared awareness

Touch-Bases are not care coordination or clinical management. They are structured conversations designed to notice and document change so nothing slips through the cracks.

EverGuide will communicate any significant changes to the family, regardless of plan level — because that is our core mission.

Family Dashboard (12 Months)

The Family Dashboard is a secure, easy-to-use online hub where family members can see updates, share notes, and track what’s happening in their loved one’s care. Instead of endless texts, scattered emails, or missed phone calls, the Dashboard keeps everyone aligned.

Core Features

  • Centralized Updates – Key notes from visits, status check-ins, or Well-Being Reports live here.

  • Shared Access – Families (and the care team, if chosen) can log in to see the same information.

  • Notifications – Alerts for new updates or changes.

  • History & Records – Keeps a running log of reports and check-ins, so families don’t lose track of what’s been said.

  • Customization – Families decide who gets access and what level of detail they see.
