The Transition Phase is designed for periods when care is changing quickly and families need closer guidance.
Over six months, Family Plus includes:

One Advance Care Planning conversation
A facilitated discussion to clarify Patient Priorities Care and guide decisions during a time of increased complexity. -
Structured Touch-Bases over six months
45-minute conversations that adjust to the pace of change:-
weekly during the first month

bi-weekly during months two and three
-
monthly during months four through six
These Touch-Bases focus on noticing and documenting meaningful changes, not clinical care.

Up to four Family Alignment Sessions
Facilitated family conversations to support shared understanding, clarify roles, and address moments when alignment matters most. -
Family Dashboard with trends and tracking
A shared space that captures observations over time, helping families recognize patterns rather than react to isolated events. -
Deeper personal involvement
Family Plus includes higher-touch involvement by James Kraft, PhD, with in-person visits available within a 45-mile radius when appropriate.
