ACA provides end to end Advance Care Planning (ACP) by combining a digital platform that removes the barriers to effective communication with patients together with experienced, certified ACP Specialists trained in the art of difficult conversations. Finally, ACA utilizes a Cloud Based Service Platform that allows providers much needed accessibility enabling them to locate and access ACP documents and portable medical orders whenever and wherever needed. Our process allows us to track and report every ACP session and report back to providers enabling them to meet Medicare mandates to do more ACP.
